Many of the spammers are automated. If you can add a custom registration field that would be unique and not programmed into the spam bot. You could ask something like the color of there eyes or shoe size for example.

I have learnt from past experiences that sometimes the accounts are manually made. The only effective way that I've found is to use e-mail activation as they use fake emails... Another good way to is promote every good, active user into a moderator

Default Moderators

That's why forum moderators are so important. Usually the forum owner is only one person who can't stay logged onto his/her forum 24/7. Many forum owners would approach the most active members and ask them to be mods, either for free or pay them to do it. I've seen forums that have members who are so passionate about the forum topic that there was always people volunteering to be moderator. They had so many volunteers that they asked members to vote to pick out moderators! And they had a moderator responsible for each section of their forum. This way each mod would only monitor their own section by logging in like once a day at least, so that any spam could be deleted quickly. This is the best arrangement I've seen for forum monitoring.

THE BEST WAY

in any forum either phpbb, vbulletin or any other forum
just use additional field of custom question while registration, or you can do same for posting.

i've done same and really telling u that i've no moderators

It does work but it doesn't have a big audience since most people hate spam. It works for newbies, but i hate spam and would do anything to stop it.
