Yes, you have to be signed into Google for the +1 button to show up. Google says that it currently will not affect search rankings, but as much as Google is pushing its social network, I'm sure it will be incorporated at some point.

Basically, the +1 button seems to be an attempt for Google to allow you to see what articles and blogs your Google+ friends are reading. Those stories can be found in the spotlight section of Google news.
