Large Breasts, Flat Nipples...Tips?

So here are my questions:

1) Any tips for large breasted/flat nipples? In particular, how do I get enough of my breast into his mouth? All the books say to make sure enough gets in there, but they don't say how if you have a big breast and your kid has a small mouth.

2) What should I do for the bleeding nipple? The books all say do nothing and continue to nurse and use lanolin. Other than that, I am not sure what to do.

3) Does anyone have some tips on how to nurse lying down? Or a good video clip that has instructions?

wear nipple shells in between nursings to help "pop out" your nipple.

consider a nipple shield with help of a lactation consultant if you can' get the hang of latching deeply.

nipple healing warm water soaks, or warm saline solution soaks, air dry, lanolin creme. switch positions each feeding so a different part of the nipple gets the direct contact of mouth, feed on one side per feeding if possible to give the other side time to heal. motrin for pain relief, or the meds from childbirth.
